Cost: $13.00 each
Proceeds benefit Braeside Elementary School PTO
Winter is coming and with winter comes cold nights and snow days. To get ready, order coupons
to stock your freezer with Lou Malnati’s frozen pizzas. An easy dinner for everyone and loved by all.
When: October 28-November 23
How to Order: Fill out THIS ORDER FORM and email to Carly or just send an email to
Carly at carlymjones13@gmail.com with your order.
How to Pay: Send a check made out to Braeside Elementary School PTO and mail to Carly
Jones, 365 Oakland Dr, Highland Park, IL 60035 or through Zelle at braesidepto@gmail.com.
All payment must be received no later than November 23, 2020.
Who: Braeside families and friends. Students should feel free to get friends and family to order if they would like.
The coupons will be delivered to family houses in early December. If you had friends and family
order as well, those will also be delivered to the Braeside family to distribute.

Pizza coupons may be redeemed at any area Lou Malnati’s location for one 9” FROZEN pizza
(sausage, pepperoni, cheese, or spinach). Offer subject to restaurant availability. Coupon has no
cash value and may not be used toward the purchase of any other items. Coupons will be delivered
